Agreement Between Hospital And Diagnostic Centre

In view of the reciprocal obligations and benefits provided by the agreement for professional medical and administrative radiology services between [hospital] and [contracting]) and as a worker, representative, partner or current shareholder of the contractor, the service provider under the agreement, I agree to be personally bound by the terms of the agreement and, in particular, to the provisions contained in Section 2.11 I have read these provisions personally and I understand their terms and conditions. 2.2.5 Representations of the contractor.
